Compliance with PHIPA

At Vampire Nurse on Wheels, we are committed to protecting your personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA).

   • We collect only the personal health information necessary to provide safe and effective nursing services.

   • This may include information such as your name, date of birth, health card number, contact information, health history, requisitions from your physician/nurse practitioner, and details related to your care.

2. Use and Disclosure of Information

   • Your information will only be used to provide nursing services, communicate with your healthcare providers as necessary, and ensure safe continuity of care.

   • We will not share your information with anyone without your consent, except where required or permitted by law (e.g., in cases of mandatory public health reporting).

3. Storage and Security

   • Personal health information is securely stored, whether in paper or electronic form, with safeguards to prevent unauthorized access, loss, or theft.

   • Access to your information is limited to authorized staff directly involved in your care.

4. Client Rights

   • You have the right to access your personal health records, request corrections, and be informed about how your information is being used.

   • You may withdraw consent for the use or disclosure of your information, subject to legal and professional obligations.

5. Retention and Disposal

   • Health records are retained for the period required by law and securely destroyed once no longer needed.

6. Accountability

   • Vampire Nurse on Wheels has a designated Privacy Officer responsible for ensuring compliance with PHIPA.

Privacy & Safety

At Vampire Nurse on Wheels, your privacy and safety are our top priorities.

• Privacy (PHIPA): We follow Ontario’s Personal Health Information Protection Act (PHIPA) to keep your personal health information safe, secure, and confidential. Your information is only used to provide nursing services and is never shared without your consent, unless required by law.

• Duty to Report: As registered nurses, we are legally required to report certain concerns to protect you and the community. This includes situations such as suspected child abuse or neglect, abuse of vulnerable adults, unsafe healthcare practices, or other risks to safety. Reports are made only when required by law and limited to the necessary information.

All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with or obtained by any third parties.
